Analysis narrative
The pilot reported to the law enforcement officer that the during takeoff from a road, shortly after it became airborne, the right wing struck a street sign. Subsequently, the airplane impacted terrain and came to rest upright, which resulted in substantial damage to the aft fuselage. Despite multiple requests, the pilot did not submit the National Transportation Safety Board Pilot/Operator Aircraft Accident/Incident Report Form 6120.1.
